Accommodative Vision Therapy 调节视觉疗法

bifocal glasses for children眼球吸收光线,然后对其进行加工处理,无论是看近处景物还是远处景物,这一过程都非常相似。不同之处是眼球周围的肌肉组织如何对晶状体—这一聚焦装置进行调整。对看近处景物的聚焦调整称为调节。当看远处景物(20英尺或6米远)时,双眼视轴是平行的,远处物体反射的光线进入眼球,聚焦在视网膜或是眼球后的感光组织上。当视线从远处移回近处时,两眼必须改变聚焦。两眼视轴要转向内侧,使两眼视轴固定于被注视物体,此种作用称为眼的集合。这一聚焦变化只发生于视线从远处移回近处时。当视线离开近处看向远处时,双眼眼轴打开(发散调节),再次处于平行状态。从集合和调节到双眼视轴平行,没有调节产生的这一过程称为发散。集合和发散过程由眼球周边的肌肉组织控制。这些肌肉组织与控制晶状体聚焦的肌肉交织在一起,集合时使晶状体聚焦,发散时使晶状体不调节。视网膜传递的形象定位信息和眼球核传递的眼球运动信息决定了眼球的集合还是发散,向左看还是向右看,向上看还是向下看。如果你的左边有物体经过,眼球核将会收到信号,再传给眼部肌肉一个向左看的信号,让影像投射在视网膜上与左边想符合的区域,从而形成最清晰的影像。眼球内部的肌肉控制调节及不调节的过程。聚焦形成时,大脑会产生查看物体的意识,眼的集合开始。大脑是怎么知道你正在看近处物体的呢?当你面前的物体被拿近时,物体投射在视网膜上的影像变大,变模糊,这轻微的模糊刺激聚焦装置开始启动。物体的接近和模糊刺激了大脑中的集合或调节细胞核。视觉系统把物体接近和模糊的信息传输给负责调节的眼球核。神经冲动通过调节或集合途径传输,引起眼睛晶状体凸度在眼球内部肌肉控制下增大。凸度的增大,增强了聚焦能力,使聚焦处于近处,引发眼的调节与集合。当物体离你远去时,视网膜上的影像缩小,对眼睛的集合和聚焦的刺激减小,眼内肌肉使晶状体凸度减小,从而使聚焦能力减弱,使聚焦处于远处。眼睛的集合与调节,发散与不调节相互依存,缺一不可。

调节视觉疗法试图解释眼睛的调节集合系统中的各种各样的误差。到目前为止,没有长期的随机双盲对照临床研究来证明这一疗法对控制或降低近视的有效性。
